Referrals and funding pathways
We accept referrals from multiple sources, including:
- NHS Continuing Healthcare (CHC) — supporting eligibility for funded care where criteria are met.
- GPs, hospital discharge teams and social care professionals
- Self‑referrals or family referrals
Our team can help guide you through CHC referral routes and support discussions with clinical teams.
Supporting Transition:
Ages 16 and Up
As part of our expanding service, U&I Care also supports young people aged 16 to 18 who are transitioning from education into adult life or care. This critical stage can be overwhelming for families so our approach ensures stability, empathy, and a focus on the individual’s goals.
We work closely with families, schools and professionals to create a bespoke care plan that supports independence, education, wellbeing, and personal growth.
